St. Croix Hospice Earns Highest Quality Connections Recognition for Sixth Consecutive Year
MENDOTA HEIGHTS, Minn. — St. Croix Hospice has once again earned the highest level of recognition in the National Alliance for Care at Home’s Quality Connections program, closing all four rings for 2026.
This marks the sixth consecutive year St. Croix Hospice has achieved the program’s highest honor, earning all four rings every year since Quality Connections began in 2021.
Quality Connections is a national program designed to support hospice and palliative care providers in delivering high-quality care through continuous quality improvement. The program is structured around four key areas: education, application, measurement and innovation. Participating organizations complete activities and meet goals within each area, with closing all four rings representing the highest level of achievement within the program.

Quality Connections originated through the National Hospice and Palliative Care Organization, which joined with the National Association for Home Care & Hospice to form the National Alliance for Care at Home. The program provides participating organizations with education, resources and quality improvement tools designed to support excellence across the care-at-home continuum.
