The St. Croix Hospice BridgeCare program™ enhances the quality of care provided on the weekend by bridging the gap in care that can occur for hospice patients admitted from Friday to Monday. The BridgeCare program™ provides a daily RN visit each day of the weekend for these new patients and schedules a Monday St. Croix Hospice RN Case Manager visit.

When a patient makes the transition to hospice care they and their loved ones may have more questions and need extra support, information and education in those first few days during their hospice admission process. Our BridgeCare program™ ensures patients who are admitted on the weekend will have the daily support they need at the beginning of their hospice journey.
