North Star Dementia Program™
Proactive symptom management for hospice patients with Alzheimer’s and dementia-related disorders to provide a safe environment and improved quality of life for patients and caregivers.
Voyage Vigil Program®
When signs and symptoms indicate a patient is approaching their final days, we increase care team presence at the bedside. The Voyage Vigil team may include our registered nurses, hospice aides, social workers and chaplains.
As part of the St. Croix Hospice North Star Dementia Program staff are Dementia Care Certified by the Crisis Prevention Institute (CPI).
