What Does Hospice Care Include?
Hospice surrounds patients and families with a team focused on comfort, quality of life and the needs that come with serious illness.
Depending on each patient’s needs, care may include:

Specialized Care for Every Stage of the Journey.
No two hospice journeys look exactly the same. The care St. Croix Hospice provides is designed to meet the unique needs of patients and families at every stage. Our specialty care programs provide additional support at key points along the way — from the first days of hospice care to the final moments — and for specific diagnoses so patients and families have the care, guidance and reassurance they need as circumstances change.
Weekend admissions. Daily nursing support.
With BridgeCare™, patients admitted over the weekend receive an RN visit each day through the weekend and an RN Case Manager visit on Monday, providing added clinical support and guidance during the first days of hospice care.
Dementia-certified care. Proactive symptom management.
With the North Star Dementia Program™, Dementia Care Certified staff proactively manage symptoms related to Alzheimer’s and other dementias, helping create a safer environment and improve quality of life for patients and caregivers.
Final days. Increased bedside presence.
With the Voyage Vigil Program®, care team presence increases at the bedside when signs and symptoms indicate a patient is approaching their final days, providing additional support for patients and families when they need it most.
