Dr. Jeffrey Hoffmann

November is National Hospice and Palliative Care Month, a time to honor the dedicated professionals who bring comfort, dignity and hope to patients and families facing serious illness. Among these compassionate leaders is Dr. Jeffrey Hoffmann, Medical Director for St. Croix Hospice’s Northeast Iowa branches, whose career reflects a lifelong commitment to service, expertise and human connection.
After completing his residency, Dr. Hoffmann practiced family medicine for many years in rural Iowa. During this time, he also was the hospice Medical Director in Prairie du Chien, Wisconsin, for about 10 years. Throughout these years, he guided patients and families through life’s most meaningful moments, blending clinical excellence with genuine empathy.
In 2024, Dr. Hoffmann extended his mission of care internationally, spending a year in New Zealand providing family medicine and hospice services to the indigenous Māori population at the Te Hiku Hauora clinic. The experience deepened his understanding of culturally sensitive care and strengthened his belief that “hospice isn’t about death; it’s about living until we die.”
When he joined St. Croix Hospice in April 2025, Dr. Hoffmann returned home to Iowa, reconnecting with the community he has served since the 1990s and with the family roots that continue to inspire his work.
“I’ve cared for patients of all ages and life stages throughout my career. Many of my current patients are familiar faces from years past, which makes my work deeply personal,” said Dr. Hoffmann.
As Regional Medical Director, Dr. Hoffmann oversees admissions, collaborates with clinical teams and partners closely with primary care providers to ensure each patient receives holistic, compassionate care.
Dr. Hoffmann embodies the spirit of National Hospice and Palliative Care Month. His expertise, compassion and dedication have touched countless lives, both locally and globally. As we honor hospice professionals this November, Dr. Hoffmann stands out as a leader who reminds us that hospice care is about living fully, cherishing every moment and supporting each other through life’s most profound transitions.
